OC Roller Girls and OC Roller Skatesinvite Troop 306, Service Unit 3 & 4 Girl Scouts and friends toSkate with the Roller Derby GirlsSunday June 9th , 2013 _______________________________________________________________________Details: 9:00am Check In9:30am-10:30am Skate Event-We will teach basic, non-hitting skate skills11:00am Awards, snacks and photo opportunities What to Bring: All skating participants will have to bring and wear quad skates and full protective gear (knee pads, elbow pads, wrist guards & mouth guard. Bring a bottle or two of water to drink. We must have a copy of your personal medical card to allow you to skate at the facility. Cost:
$20 per skater preregistered / $25 at the door***If you plan to register at the door- please email us at firstname.lastname@example.org so we can make sure to have enough trainers for you.Non skating Girl Scout members are free welcome to watch!
Includes workshop & a ticket to see the OC Roller Girls game on April 13 in Huntington Beach. Additional April bout tickets will be available at the event for a discounted rate. WAIVER:By registering you agree to the following:ASSUMPTION OF RISK, WAIVER, AND RELEASE FROM LIABILITY In consideration of the use of the property, facilities and/or services of the OC Roller Girls or any AUXILIARY ORGANIZATIONS (Auxiliaries) participating or sponsoring OC Roller Girls Programs, including travel related thereto, the participant agrees as follows: RISK FACTORS: The participant understands and acknowledges that the use of equipment, facilities and services provided by the OC Roller Girls’ programs (roller derby, roller skating, other physical sports, weight and cardiovascular training, aerobics, stretching, and any other programs and services sponsored by the OC Roller Girls or its associated Auxiliaries) involves risks such as, but not limited to, the following which might result from the use of equipment or facilities, from the activity itself, from the acts of others, or from the unavailability of emergency or emergency medical care. RISK OF PROPERTY DAMAGE, BODILY INJURY, AND POSSIBLE DEATH.ASSUMPTION OF RISK: The participant ASSUMES ALL RISKS THAT ARISE OUT OF THE USE OF THE EQUIPMENT OR FACILITIES, THE ACTIVITY ITSELF, THE ACT OF OTHERS, OR THE UNAVAILABILITY OF EMERGENCY CARE, including but not limited to, those RISK FACTORS described in section 1 above.PREREQUISITE SKILLS AND TRAINING: The participant acknowledges that he or she has the requisite skills, qualifications, physical abilities, and training necessary for proper and safe use of the equipment and facilities and to participate in the activity itself. The participant agrees that if he or she has any questions as to what skills, qualifications or training is necessary to properly use the equipment, facility, or to participate in the activity itself, then they shall direct such questions to the appropriate training team member. The participant willingly agrees to comply with the stated and customary terms and conditions for participation. RELEASE: The participant RELEASES the OC Roller Girls, all members of the OC Roller Girls, any person participating in activities in association with OC Roller Girls, and the OC Roller Girls' affiliated entities, any agent, owner, employee or representative of the OC Roller Girls or any such entity or any successors in interest, heirs and assigns of any of the foregoing released persons and entities and agrees NOT TO SUE them on account of or in conjunction with any claims, causes of action, injuries, damage, cost of expenses arising out of the activity, including those based on death, bodily injury or property damage whether or not caused by the acts, omissions or other fault of the parties being released.WAIVER: The participant certifies that they have read the following provisions of California Code of Civil Procedure Section 1542 and agrees to waive the protection afforded by that statute. Section 1542 provides: A general release does not extend to claims which the creditor does not know or suspect to exist in his favor at the time of executing the release, which if known by him must have materially affected his settlement with the debtor. By registering, the participant agrees to waive the application of California Civil Code Section 1542. The participant understands and acknowledges that the significance and consequence of this waiver of California Civil Code Section 1542 is that even if he or she should eventually suffer additional damages arising out of the claims released herein, the participant will not be able to make any claim for those damages. Furthermore, the participant acknowledges that he or she consciously intends these consequences even as to claims that the participant does not know exist, and that, if known, would materially affect the participant’s decision to execute this Agreement, regardless of whether the participant’s lack of knowledge is the result of ignorance, oversight, error, negligence or any other cause.INDEMNIFY AND DEFEND: The participant agrees to INDEMNIFY AND DEFEND the OC Roller Girls, all members of the OC Roller Girls, any person participating in activities in association with OC Roller Girls, and the OC Roller Girls' auxiliary organizations, the officers, employees and agents (hereinafter jointly referred to as "indemnitee") of each against, and hold them harmless from any or all claims, causes of action, damage judgments, costs or expenses, including attorney fees which in any way arise from the activity or this agreement which include but are not limited to damages to or destruction of any property of the indemnitee, of any others, injury or death of the participant or anyone else or any liability arising from the act or negligent act of the indemnitee, the participant or anyone else.ATTORNEYS’ FEES: In the event a dispute arises out of this Agreement, the prevailing Party shall be entitled to recover its reasonable attorneys’ fees and costs incurred in the proceeding, in addition to any other relief which the prevailing Party may be entitled.PAY: The participant agrees to pay for any or all damages to any property or indemnitee caused by the participant either negligently, willfully, or otherwise.RIGHT TO REVIEW BY INDEPENDENT COUNSEL: The participant acknowledges that he or she has had the right to consult independent counsel of their choice before executing this waiver and release, and that they understand and appreciate the terms and the binding effect of this waiver and release.EMERGENCY TREATMENT CONSENT: The participant in the subject activity, hereby consents to medical treatment in a medical emergency where the participant is unable to consent to such treatment.INSURANCE: The participant understands that the OC Roller Girls and its auxiliaries do not carry participant insurance. The participant is encouraged to have a physical examination and is required to maintain health insurance prior to any and all participation.ACKNOWLEDGMENT: The participant has read and understands this agreement and realizes it relates to surrendering valuable legal rights and does so freely and voluntarily.I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY REGISTERING AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. I attest that I am physically fit and have been trained for this activity. I also waive and release the use of my photograph or likeness for any reason or purpose. I WANT TO PARTICIPATE IN THIS HAZARDOUS SPORT! I AGREE TO ASSUME FULL RESPOINSIBILTY FOR ALL INJURIES AND MEDICAL EXPENSES INCURED WHILE PARTICIPATING OR ATTENDING THE EVENT OR PROGRAM.
