The federal Occupational Safety and Health Administration fined Northeast Energy Management Inc. $5,965 for a drilling rig fire in Clarion County that burned a worker in September 2007. The company also paid a $4,000 fine for a drilling rig fire in Charleroi in November 2008.
OSHA and other investigators have yet to determine the cause of Friday's explosion in Indiana Township that killed Andy Yosurak Jr., 56, of Creekside, and Kevin Henry, 46, of New Florence.
The state Department of Environmental Protection, OSHA, the county fire marshal and the well's owner, Huntley & Huntley Inc. of Monroeville are still investigating the cause of Friday's explosion near Rich Hill Road.
